Rhinoplasty

What is rhinoplasty?  

Rhinoplasty can enhance facial harmony by augmenting the proportions of your nose. It can also correct breathing obstructions caused by structural defects in the nose.  

Rhinoplasty surgery can change:

Rhinoplasty surgery steps  

Rhinoplasty surgery includes the following steps: 

Step 1 – Consultation 

Our surgeon will meet with you to evaluate your eligibility for your requested surgery, to discuss your goals and to advise you on the expected outcome.  You will be required to complete a health questionnaire which will be reviewed during your consultation. It is very important to understand all aspects of your rhinoplasty procedure, so be sure to ask questions. It is natural to feel some anxiety and preoperative stress and our team will make every effort to put your mind at ease. 

Step 2 – Hospital admission & Surgery prep  

On the morning of your surgery, you will be admitted to the hospital and our surgeons will go over your health questionnaire, take your vital signs and do a full blood panel to ensure you have no conditions barring you from surgery. The surgeon will come and mark the areas that will be treated and photographs will be taken. 

Step 3 – Anesthesia 

Once you have been cleared for surgery, medications are administered for your comfort during the surgical procedure. The choices include local anesthesia, intravenous sedation, and general anesthesia. Our surgeons will recommend the best choice for you. 

Step 4 – The incision 

Rhinoplasty is performed either using a closed procedure (incisions hidden inside nose), or an open procedure (external incision is made across the strip of tissue that separates the nostrils). 

Step 5 – Reshaping the nose structure 

The underlying structure of the nose can be reduced by removing cartilage and bone tissue. In some cases, the nose may require the addition of cartilage or bone grafts.  Typically, cartilage is taken from the septum for this purpose.  

* Any breathing obstructions due to a deviated septum, can be corrected by re-aligning the nasal structure in conjunction with the rhinoplasty procedure. 

Step 6 – Closing the incision 

Once the underlying structure of the nose is augmented to the desired shape, nasal tissue and skin is repositioned and incisions are closed. Incisions may be placed in the creases of the nostrils to alter their size. 

Step 7 – Post Surgery 

Immediately following your rhinoplasty procedure, our surgeons will fit you gauze packing inserted into the nostrils and splints and bandages externally, to support the structure of your nose through the initial healing period.  You will then be taken to the recovery ward for about two hours prior to being moved back in your room. On staff doctors and nurses will monitor you overnight and once cleared by our surgeon, you will be released the following afternoon and accompanied back to your hotel. 

Step 8 – You will see our surgeon the following day and up to 3 follow up visits post-surgery.  The gauze packing and splints will support your nose as it begins to heal and will typically be removed after a few days. 

Although rhinoplasty results are noticeable immediately, full results will be noticeable once the swelling, bruising and will continue to improve up to one year following surgery.

Breast Lift

  • A breast lift — also known as mastopexy — is a surgical procedure performed by a plastic surgeon to change the shape of your breasts.

    Why it’s done

    As you get older, your breasts change — losing elasticity and firmness. There are many causes for these kinds of breast changes, including:

Facelift

What is a facelift?

A facelift, or rhytidectomy, is a surgical procedure that improves visible signs of aging in the face and neck, such as:

  • Sagging in the middle of your face
  • Deep creases below the lower eyelids
  • Deep creases along the nose extending to the corner of the mouth
  • Fat that has fallen or has disappeared
  • Loss of skin tone in the lower face that creates jowls
  • Loose skin and excess fatty deposits under the chin and jaw can give even a person of normal weight the appearance of a double chin

The loss of youthful contours in the face can be due to a variety of factors, including heredity, gravity, environmental conditions, and stress.

Rejuvenation procedures typically performed in conjunction with a facelift are brow lift, to correct a sagging or deeply furrowed brow, and eyelid surgery to rejuvenate aging eyes.

What a facelift won’t do

As a restorative surgery, a facelift does not change your fundamental appearance and cannot stop the aging process.

A facelift can only be performed surgically; non-surgical rejuvenation treatments cannot achieve the same results but may help delay the time at which a facelift becomes appropriate and complement the results of surgery

In general, good candidates for a facelift include:

Facelift procedure steps

A facelift procedure includes the following steps:

Step 1 – Anesthesia

Step 2 – The incision

Depending on the degree of change you’d like to see, your facelift choices include a traditional facelift, limited incision facelift or a neck lift.

A traditional facelift incision often begins in the hairline at the temples, continues around the ear and ends in the lower scalp. Fat may be sculpted or redistributed from the face, jowls and neck, and underlying tissue is repositioned, commonly the deeper layers of the face and the muscles are also lifted. Skin is redraped over the uplifted contours and excess skin is trimmed away.

A second incision under the chin may be necessary to further improve an aging neck. Sutures or skin adhesives close the incisions.

Traditional Facelift

An alternative to a traditional facelift uses shorter incisions at the temples, continuing around the ear and possibly within the lower eyelids or under the upper lip.

Limited Incision

Sagging jowls, loose neck skin and fat accumulation under the chin may be corrected with a neck lift. The neck lift incision often begins in front of the ear lobe and wraps around behind the ear ending in the lower scalp.

Step 3 – Closing the incisions

Once healed, the incision lines from a facelift are well concealed within the hairline and in the natural contours of the face and ear.

Step 4 – See the results

The visible improvements of a facelift appear as swelling and bruising subside. Your final result should not only restore a more youthful and rested appearance, but also help you feel more confident about yourself.

Facelift recovery

During your facelift recovery, a bandage might be placed around your face to minimize swelling and bruising once your procedure is completed. Thin tubes may be present to drain any excess blood or fluid that may collect under the skin.

You will be given specific instructions that may include how to care for the surgical site and drains, medications to apply or take orally to aid healing and reduce the potential for infection, specific concerns to look for at the surgical site or in your general health, and when to follow up with your plastic surgeon.

Gastric Sleeve

Sleeve gastrectomy is a relatively new bariatric operation, and currently comprises 10-20 percent of bariatric operations performed in the US: This number is expected to rise in coming years. Sleeve gastrectomy is a safe and effective operation. Your surgeon will help you decide if this is the right operation for you.

Sleeve gastrectomy involves removal of the majority of the stomach, leaving a narrow “sleeve” of stomach that limits food intake much like a gastric pouch, but without the need to rearrange intestinal anatomy. Sleeve gastrectomy thus restricts the amount of food a person can eat and provides a feeling of fullness.

Weight loss after sleeve gastrectomy ranges from 50-60 percent of excess weight in most patients, similar to gastric bypass. More than 80 percent of patients will experience marked improvement in related diseases such as diabetes, sleep apnea, high blood pressure, fatty liver disease, osteoarthritis, and other diseases

  • The rest of the gastrointestinal tract anatomy is not altered, so the food ingested follows the normal course. This results in less chance of developing lack of vitamins and minerals and eliminates ‘dumping syndrome’.
  • The procedure significantly decreases hunger sensations by removing the part of the stomach that produces the main stimulating hormones.
  • It minimizes the chance of developing ulcers.
  • There are normally fewer office visits than with gastric banding, since there is no need for adjustment of the size of the band.

Gastric Band

The adjustable gastric band is made of soft silicone and is equipped with a firmly attached inflatable balloon. This balloon is connected by a tube to an access port.

The band is positioned around the upper part of the stomach by keyhole (or laparoscopic) surgery so as to create a small pouch. This small pouch lies above the band and has a capacity of 15-20mL. The remaining stomach lies below the band. The band helps patients reduce weight by 2 mechanisms:

  • By reducing the amount of food that an individual wants to eat by inducing a feeling of fullness after only half a cup of food; and
  • By slowing down the emptying from the pouch above the band, thereby prolonging the feeling of fullness

The size of the pouch outlet is adjusted by adjusting the volume of fluid in the balloon through the access port. The access port is situated under the skin usually in the upper abdomen and is easily accessible by the treating doctor with a special needle.

Since the insertion of the adjustable gastric band does not involve any stapling, resection or shortening of any part of the stomach or intestines, the function of the digestive system remains intact. All ingested food will still be absorbed by the body the same way that it was before the operation. Weight loss is achieved by helping induce early feeling of satiety after eating considerably smaller amounts of food than before the surgery. It is most important to avoid foods of high caloric content, such as ice cream, custards and chocolates.

The Operation

Insertion of the adjustable gastric band is usually done via laparoscopic approach.

Five or six small incisions are made in the abdomen to introduce a camera and instruments and the adjustable gastric band is positioned at the upper end of the stomach.

Once the band is placed in position, the connecting tube and access port are inserted and secured to the upper abdominal wall.

All incisions are finally closed using absorbable sutures which do not need to be removed.

How much weight can you expect to lose?

The weight will be lost gradually and should commence immediately after surgery. The amount of weight loss will depend partly on the amount of fluid injected into the balloon to provide a feeling of satiety. More importantly, the amount of weight loss in the long term will depend on the patient, i.e. on how carefully the patient follows the recommendations regarding eating habits, diet and exercise. The recommended rate of weight loss we would like to see is 0.5 – 1.0 kg per week. On average it is expected that 50-60% excess weight will be lost long term.

This also means a long-term commitment to follow up by your bariatric clinic as data clearly shows the importance of follow up for successful weight loss.
